Medical Forms

CAP revised its medical forms. The old CAPF 60 is not longer used. There are two new forms that apply to our activities. For cadets under 18, there is the CAPF 161 - Emergency Information Form and the CAPF 163 - Permission for Provision of Minor Cadet Over-the-Counter Medication Form. For seniors and cadets 18 and older, only the CAPF 161 is needed.

The CAPF 161 requires a copy of the front and back of your or your parents insurance card.

ECS Weekly Meeting Information

Evergreen Composite Squadron (ECS) meets every Monday at the Evergreen Lutheran Church, 5980 Highway 73, Evergreen CO. Meetings begin at 6:30 PM and end at 9:00 PM.  Times are subject to change.

Visitors are always welcome!

CAP Bumper Stickers are Available!

Evergreen Composite Squadron is offering CAP Bumper Stickers as part of a unit fundraising project. The bumper sticker is approved by Colorado Wing and by CAP National Headquarters. For ordering information, click on the "Order Bumper Stickers" link in the Site Navigation Pane on the left.
